Runswithscissors, you know what? The Seiko small diver's watch posted by Jonathan is not very big and very likely just at your bogey of 38mm. I usually  surf http://www.chronograph.com for lots of Seiko choices. I am very sure you can find a small ladies diver's watch there. I recall a buddy of mine was able to get a small diver's watch for his wife. Check out that site and zero in on the ladies' versions.

Incidentally, if you are able to get a Seiko diver's watch for your wife, you can also change the steel braclet or rubber strap to a Nato strap which can come in colors ladies will love, aside from the usual black.
