The moral of this story is, don't use a 22mm Zulu on 24mm lugs.
This is tragic, Willieboy.
I was trying to play out that scene in my mind and you're right...using wrong sized zulus or natos doesn't guaranty the head stays where it is. I think your case was a very rare one wherein both spring bars or pins were twisted off their holes simultaneously. Hope the head fell on the bed or just the grass.
